Do it online!
From April 2016, access to the coded information in your medical records is now available to view online. This information includes your immunisation history and test results, as well as any conditions you have presented to the GP or Nurse with (asthma for example).
In line with national guidance if you require access to your medical record facility we will require the following:
- completion of an application form (also available from Reception)

- photo ID (driving license or passport) - we will keep a copy of this for records
- proof of address (bank statement or utility bill) - we will keep a copy of this for records
Any patient requesting access will need to attend surgery in person in order that we can identify each individual, so we will not be able to accept applications from other family members on your behalf. This is to ensure we are maintaining the confidentiality of your records to the best of our abilities.
Patients who had registered for online access prior to April 2016 will already be using the appointment booking facility, secure messaging and prescription requesting. However, to have access to the records function, you will be asked to provide the above documents.
New patients requesting access for the first time, upon providing us with the above, will be given access to the appointments facility and secure messaging only until the authorisation process is complete.
A Doctor will review the application and your records and give their authorisation. This will show when you log into the system as these functions will be available to you. Due to the number of requests we are receiving, please note this can take 4-6 weeks, however, we will endeavour to get authorisation sooner where possible.
