HOW DO I....
Change My Contact Details?

Please notify us as soon as possible if any of your personal details change (ie address, telephone number etc). You can do this by one of the following methods:

• Come into the surgery to collect and complete a form.

• Click on the link on our website to download and complete the form. Then either drop this form off at our reception desk or post it to us.

• If your surname has changed because you are married/divorced/changed by deed poll etc, we will require sight of official documentation to prove this (eg passport, marriage
certificate etc).

You may be contacted by our Registrations Administrator if there are any queries, or you have moved out of our catchment area (click her to see map). This will be done by telephone or letter.

MOBILE PHONES

We do not allow the use of mobile phones in the surgery.
